Patient Information We accept WCB, ICBC, RCMP and DVA patients. No referrals are necessary. We have a selection of products for sale including ice packs, pillows, back support cushions, Thermotex infrared heat pads, and orthotics.

Patient Forms To help our patients here are some common forms that need to be filled in. Health History Form - required to be filled in by all patients. WCB Form - this is required to be filled in for all WCB claims. ICBC Form - this is required to be filled in for all ICBC claims.
